This information is being gathered so that our volunteers at Citizens Kids can better serve your child/children while they are participating in our Sunday morning programs. It is assumed that a parent is present in the building while the child is in the class. Therefore, a parent would be available to give consent for assessment and/or treatment in the case of a medical emergency.
